Immediate Response (0-2 jam):
1. Secure the Scene
- Isolate area dengan barricade
- Preserve evidence - jangan move equipment
- Take photos dari multiple angles
2. Provide First Aid/Medical Attention
- Prioritas #1 adalah injured person
- Call emergency services jika perlu
3. Notify Authorities
- Inform supervisor, safety officer, management
- Untuk fatality/serious injury: notify Disnaker dalam 2x24 jam
Investigation Process (2-48 jam):
4. Form Investigation Team
- Safety officer, supervisor, operator (jika able), maintenance, witness
5. Gather Evidence
- Physical: Equipment condition, load, terrain, weather
- Documentary: SIO, maintenance log, training record, JSA
- Testimonial: Interview operator, witness, supervisor
6. Reconstruct Sequence of Events
- Timeline detail: apa yang terjadi step-by-step
7. Root Cause Analysis
- Use 5 Whys technique atau Fishbone Diagram
- Identify immediate cause, contributing factors, root cause
Example 5 Whys:
- Why accident? - Crane tip-over
- Why tip-over? - Overloaded
- Why overload? - Operator tidak cek load chart
- Why tidak cek? - Tidak trained proper
- Why tidak trained? - Root: Training program inadequate
8. Develop Corrective Actions
- Address immediate, contributing, dan root cause
- Assign responsibility dan deadline
9. Document & Report
- Incident report lengkap dengan findings dan recommendations
10. Follow-up
- Verify corrective actions implemented
- Share lesson learned dalam TBT
